对TMJ问接损伤后MMP一3的拮抗作用研究

[摘要] 目的:对家兔TMJ(Temporomandibular joint,TMJ)间接性损伤后于其关节腔内注射5%碳酸氢钠,研究碱 

性试剂对细胞外基质的作用。方法:应用关节病理损伤指数、关节软骨含量和关节软骨MMP-3(matrixmetalloprotein- 

ases,MMPs)免疫荧光染等观察指标来检测实验组和对照组动物。结果:4周及8周实验组动物软骨含量均高于对 

照组,两者比较差异有显著性(P<0,05);4周及8周对照组的关节病理损伤指数均高于实验组,有统计学差异(P< 

0.05);4周及8周的关节软骨MMP一3免疫荧光染积分实验组均低于对照组,差异有显著性(P<0,05)。结论:应 

用碳酸氢钠可显著提高TMJ间接损伤的关节软骨含量,降低MMP-3表达,减轻关节创伤性炎症。

Study on MMP-3 Antagonism of Injecting 5%Sodium Bicarbonate into the Articu- 

lar Cavities after TMJ Indirect Injury

[ABSTRACT]objective:To study the degradation of alkaline reagents for extracellular matrix through injecting 5% 

sodium bicarbonate into the ioint cavities after indirect injury of temporomandibular joints in rabbits.Methods:The ani- 

mals of the experimental group and the control group were detected by observation criteria such as the injury index of ar- 

ticular pathology,the contents of articular cartilage and the articular cartilage MMP-3(matrixmetalloproteinases,MMPs) 

immunity fluorescence dyeing.Results:The animal cartilaginous contents of the experimental group at the fourth and the 

eighth weeks were higher than those of the control group and there was diference between the two groups(P<0.05). 

The injury index of articular pathology of the control group at the fourth and the eighth weeks was higher than that of the 

experimental group and there was difference between the two groups(P<0.05).The articular cartilage MMP-3 immu。 

nity fluorescence dyeing of the integral experimental group at the fourth and the eighth weeks was lower than that of the 

control group and there was difference between the two groups(P<0.05).Conclusion:Using sodium bicarbonate may 

obviously enhance the articular cartilaginous contents of TMJ indirect injury,reduce the MMP-3 expression and relieve 

the joint traumatic inflammation.

从临床和动物实验研究证实,颞下颌关节创伤可以 

导致颞下颌关节骨关节病(TMJOA),严重者则可发展为 

颞下颌关节强直。由于创伤破坏了关节表面的屏障作 

用,同时关节内环境变化,软骨细胞损伤后释放多种酶 

MMP-3在颞下颌关节间接性损伤所致的骨关节病和关 

节强直中的作用,为临床提供多元治疗途径。 

1材料与方法 

1.1动物分组 

类,使胶原蛋白网络断裂,蛋白多糖降解,软骨发生不可 

逆的损害。基质金属蛋白酶家族则被认为在OA及颞 

下颌关节强直病变中软骨退行性改变直至溶解破坏的 

过程中扮演了重要角 。本研究观察碱性试剂对 

健康的3月龄大小的家兔16只(由山西医科大学 

动物实验中心提供),体重为2.5 kg一3.0 kg,随机分 

为4周组和8周组,每组8只。造模前4周和8周组 

所有实验动物的左侧TMJ为实验组,右侧TMJ为对照
