f摘要] 目的探讨双管同步换血治疗新生JIF ̄I!红素血症对机体内环境的影。方法 回顾性分析重庆儿童医院 

新生JI ̄f"}136例高胆红素血症新生儿经外周动静 双管同步换治疗的临床资料,比较换血前后血清胆红素、生化、血 

常规的变化。结果换血后血清胆红素明显下降C 0.01),同时,血清钾、钠、氯、钙离子、血小板、白细胞、碳酸氢根浓 

度均由不同程度下降(P<0.O1);血糖、血清磷有升高(J)<0.01);血红蛋白、pH则在换血前后无明显变化(尸>O.05)。结论 

外周双管同步换血治疗新生儿高胆红素血症有显著疗效,对内环境有一定影n向,但这些影响不对机体构成严重危险。

[Abstract]Objective To study the effect of exchange transfusion on the internal environlnent homeostasis of neonates with 

hyperhillirubinemia.Methods The clinical data of 136 examples of neonates with hyperhillirubinemia in hospital was 

retrospectively analyzed.All neonates were treated by peripheral arteriovenous simultaneous exchange transfusion with the 

standard of exchange transfusion.The data of seruln bihrubin,bioehemisuT,full blood examination before and after the 

exchange transfusion were compared.Results the serulll bilirubin was dramaticly decreased after exchange transfusion when 

eomparied with before exchange transfusion(P<0.01).the concentration of serum kalium,natriuln,chlorine,calcium,blood 

platelet,white blood cell and bicarbonate radical were decreased signiifcantly after exchange transusfion(P<0.O1),the 

concentration of serum glucose and phosphorus were increased obviously after replacement transfusion fP<O.O1).No 

signiifcant diferemce of hemoglobin and pH was observed before and after exchange transfusion fP>0.05).Conclusions 

Synchronizing replacement transfusion of peripheral blood was effective treatment for neonate with hyperhilirubinemia, 

which had a mild side effect on internal environment homeostasis.

病理性黄疸是新生儿期的常见病,重症高胆红 

素血症容易导致胆红素脑病,表现为智力障碍、脑 

瘫、听觉、视觉障碍等后遗症,严重影响患儿健康成 

长及生存质量,给家庭和社会增加负担。国内外大量 

研究表明外周双管同步换血是治疗严重高胆红素血 

症、预防胆红素脑病积极有效的方法。由于换血容量 

达到新生儿全身血容量的两倍【“,因此可能对患儿的 

机体内环境产生不同程度影响,本研究拟探讨换血
