口服碳酸氢钠对挤压综合征家兔肾脏的保护作用

【摘要】 目的探讨口服碳酸氢钠对挤压综合征家兔肾脏的保护作用。方法 24只健康家兔随 

机均分为4组,均固定于兔台,且c,D组挤压右后肢上部5 h后,A,c组自由饮用清水,B,D组自由饮用 

碳酸氢钠溶液24 h后,取血清和尿行生化检测,取肾组织光电镜及免疫组化形态学观察。结果 c,D 

两组BUN,Cr较A组高,cO2CP及血清Mb较A组低(均P<0,05);C组K 较A组高及尿pH较A 

组低(均P<0.05),而D组无差异(均P>0.05);D组K ,BUN及cr较c组低,而血清COzCP,Mb 

及尿pH较C组高(均P<0,05)。A,B两组结构正常;C组肾小球内皮细胞重度肿胀,足细胞足突肿 

胀、完全融合,肾小管上皮细胞胞浆空化、部分细胞崩解脱落,管型成分均质物、红细胞、细胞碎片,D组 

肾小球内皮细胞肿胀,足细胞足突肿胀、轻度融合,肾小管上皮细胞胞核水肿、核旁空泡线粒体肿胀,管 

型成分均质物、红细胞。A,B两组肾脏未见肌红蛋白管型;c,D两组肾小管中有肌红蛋白管型,主要存 

在于远曲小管,但D组较C组轻。结论 碳酸氢钠对挤压综合征家兔肾脏的保护作用。

[Absltract]Objective To investigate the protection effect of oral administration sodium bicarbon— 

ate on rabbit ren from Crush syndrome.Methods 24 health rabbits were divided into 4 groups equally, 

fixed on platforms.Further right hind limb of rabbits were crushed 5h in Groups C and D.24h after rab— 

bits draught clear water in Groups A and B while sodium bicarbonate in Groups C and D.blood--serum 

,urine and nephridial tissue were observed.Results BUN,Cr of serum in Groups C and D were higher 

than these in Group A,while C02CP(p<0,05),Mb of serum were lower;K+of serum in Group C was 

higher while pH of urine was lower than that in Group A(p<O,05),there was no difference between 

Group A and D(p<0,05),K+BUN,Cr were higher while C02CP,Mb and pH of urine were lower in 

Group D than Group C.structures of Groups A and B were normal,glomerulus andotheliocyte heavy 

swell,foot process swell and coalesce completely,endochylema of renal tubular epithelial cell cavitati— 

ons,some cell disruption,Cast were made in homogeny,akaryocyte,cell debris in Group C.glomerulus 

endotheliocyte swell,foot process swell and coalesce,endochylema of renal tubular epithelial cell cavita— 

tions,some cell disruption,Cast were made in homogeny,akaryocyte in Group D.myohemoglobin cast 

in Groups A and B were no found while these in Groups C and D were found,but these in Group C were 

less than that in Group D.Conclusions The effect of sodium bicarbonate on rabbit ren from Crush syn— 

drome was utility.

地震、车祸造成的挤压综合征,其导致死亡原因多认为是 

肾功能衰竭[1 ]。碱物碳酸氢钠既可利尿又可碱化尿液 

是否能够减轻肌红蛋白在肾小管中沉积,本实验以光电镜及 

免疫组化方法观察家兔肾组织,旨在为挤压综合征肾脏保护 

c组自由饮用清水,D组自由0.8 碳酸氢钠溶液,任其自由 

活动,解除挤压后24 h断颈处死后立刻肾脏取材。 

1.2血清生化指标测定 处死前膀胱穿刺取尿,心脏穿刺 

3 ml冷冻离心3 500(r/min,6 min),取血清0.6 ml立即送检验 

科检测,采用全自动生化检测仪,测血清尿素氮(BUN)、肌酐 

提供可靠的依据。 

1材料和方法

质、皮髓交界、髓质处组织,立刻置于2.5 戊二醛溶液和l 

锇酸内双固定后系列丙酮脱水,618环氧树脂包埋,LKBV型 

超薄切片机切片,醋酸铀和枸橼酸铅双染,JEM100--CX型
